Requirements for Foreign Legal Entities

Requirements that a Foreign Legal Entity must have up to date when completing the ACRXS Supplier Registration:

• Certification issued by the competent government authority, no more than 15 days old, regarding the legal entity’s registration.
• Certification issued by the competent government authority, no more than 15 days old, regarding the power of attorney of the legal representative (signatory).
• Valid risk insurance policies.
• Completed conflict of interest form.
